Diana Princess of Wales Care at Home Trust is an Isle of Man charity that provides high quality nursing care to patients who have a life-limiting illness or a progressive disease where curative treatment is no longer possible or appropriate and where the patient wishes to remain at home.
The care is provided in the patient's home on the Isle of Man by Registered Nursing staff, Registered Sick Children's Nurses and experienced Health Care Assistants and Nursery Nurses trained in palliative care by this Isle of Man charity. Support is delivered, both physically and emotionally, to the individual patients, their family members, friends and carers.
Respite care is offered where needed.
THE WORK THEY DO:
Hospice at Home
This service is provided free of charge to patients resident in the Isle of Man. A GP, Consultant, District Nurse, Health Visitor or Social Worker may refer the patient to the Palliative Care Specialist Team based at Hospice, Isle of Man. One of the Clinical Nurse Specialists will make a careful assessment and organise the care that is required.
The charity also supports children with life limiting conditions and their families both emotionally and practically by providing respite care in their homes.
The service is supported by the Diana Princess of Wales Care at Home Trust and is provided by Hospice Isle of Man.
